Currently the treatment of chronic hepatitis B include nucleoside analogue lamivudine (LAM), adefovir (ADV), telbivudine (LdT), entecavir (ETV) and tenofovir (TDF). The first four are the domestic pharmaceutical market, and has been widely used clinically, and TDF only in the foreign market, the country has not been used for the treatment of hepatitis B. Therefore, this article will make a brief introduction to the characteristics of the drug, clinical efficacy, drug resistance and the like.
1. TDF of PK / PD Basic Features
TDF its prodrug tenofovir disoproxil fumarate [9- (R) -2- (methoxy-phosphate) - propyl] adenine, or PMPA in) administration forms, are phosphorylated to PMPApp nucleotides. After PMPApp if incorporated into the DNA chain may terminate DNA strand, natural deoxyadenosine 5'-triphosphate is a competitive inhibitor. 2. The clinical efficacy of tenofovir
Ⅲclinical studies 102 and 103, respectively, compared TDF and ADV treatment of HBeAg-negative and HBeAg positive difference in efficacy [1-2], ADV group of patients switching to TDF continued for 48 weeks. The results showed (see Table 1), 48 weeks of treatment TDF group HBV DNA negative rate was significantly higher than ADV group (HBeAg-negative patients were 93% and 63%, P < 0.001; HBeA positive patients were 76 % and 13%, P < 0.001). HBeAg seroconversion rates similar groups (21% and 18%, P = 0.36), HBsAg seroconversion rate to ADV group (3.2% and 0, P = 0.02). 144 weeks when, TDF group 87% HBeAg-negative patients and HBV DNA 72% HBeAg positive patients < 400 copies / mL, in ADV converted to TDF group, the rates were 88% and 71%, of the total of HBV DNA negative rate were 81% and 74% for normal ALT levels, HBeAg negative rate of 34%. Visible ADV treatment group converted to TDF, more patients have been HBV DNA negative, negative rate of similar patients newly diagnosed with TDF.
3. TDF is the new nucleoside (acid) analogues, clinical studies have shown its HBV, HIV co-infection and lamivudine-resistant patients have good antiviral activity [19]. New research also shows that, TDF alone single treatment CHB infection also has good efficacy and viral inhibition. Since the anti-HBV effect of TDF is superior to adefovir, low incidence of resistance, and effective for most resistant strains of HBV. Thus, TDF has HBV infections in broad application prospects [20], many clinical studies on TDF therapy single CHB efficacy and safety, etc. Is in full swing, and for TDF in hepatic insufficiency, renal insufficiency, liver transplantation patients need further efficacy and safety of large-scale clinical studies, pregnant women and children CHB efficacy and safety also require further study.
