3. Mechanisms of action of gefitinib
(1) to compete for the Mg-ATP binding site on the EGFR-TK catalytic domain and block its signal transduction;
(2) inhibition of mitogen - activated protein kinase activation, and promote apoptosis;
(3) inhibition of tumor angiogenesis. Iressa was approved by the Japanese Ministry of Health on July 5, 2002 for the treatment of advanced NSCLC, May 5, 2003 was approved by the FDA as a third-line NSCLC treatment, the recommended dose of 250mg, PO, qd.
It is worth noting that FDA's decision was approved by Iressa just after completion of phase II clinical trials and phase III clinical trials have not yet been completed. Pharmacokinetic properties: Gefitinib is a selective epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor, which is normally expressed in epithelial-derived solid tumors. Inhibition of EGFR tyrosine kinase activity can inhibit tumor growth, metastasis and angiogenesis, and increase tumor cell apoptosis. In vivo, gefitinib broadly inhibits tumor growth in xenografted human tumor cell lines in nude mice and increases the anti-tumor activity of chemotherapy, radiotherapy and hormone therapy. Gefitinib has been shown to have an objective anti-tumor response to locally advanced or metastatic non-small cell lung cancer and to improve disease-related symptoms in clinical trials. Pharmacokinetic properties After intravenous administration, gefitinib rapidly cleared, widely distributed, with an average clearance half-life of 48 hours.
